Here are a few tips you can try:

  I will ask the 3 questions.

 I will bring a friend or family member to help me at my doctor visit.
 I will make a list of my health concerns to tell my doctor or nurse.
 I will bring a list of all my medicines when I visit my doctor or nurse.
 I will ask my pharmacist for help when I have questions about my medicines.

Your doctor, nurse, and pharmacist want you to get the information you need to care for your health. To remind yourself what you need to do and the questions you can ask at your next medical visit

Sliding Fee Scale Services

Individuals without health insurance or with high insurance deductibles may apply for Kent Community Health Center's Sliding Fee Scale Program, which offers discounted rates for most Medical and Dental services.
You must speak with a Case Manager to be approved for this program.

Eligibility for this Sliding Fee Scale Program is based on annually revised federal poverty guidelines regarding income and family size. Proof of annual income is required to enroll into the program and must be updated annually to maintain enrollment.  Patients who have medical insurance with high deductibles or reimbursement limits may want to consider applying for our sliding fee scale as well.


*Payment is expected at the time of your visit.

Acceptable forms of proof of income are copies of:

 Two most recent pay stubs
 Most Recent Federal Income Tax Return 

 Social Security/Disability Statements 
 Government issued photo identification
